Terms and Conditions
By enrolling in this program, you are agreeing to the terms, conditions and guidelines outlined.
1. I understand that the coaching services provided are intended for personal development and are not a substitute for professional advice, therapy, or counseling. I acknowledge that I am solely responsible for my actions and decisions as a result of participating in coaching sessions. The Client acknowledges that they are entirely responsible for their progress and results from the Program. [SilverLiningLash] will provide assistance and guidance, the Client's active participation is crucial for success. The Company does not make any verbal or written promises regarding the Client's performance. The Client understands that due to the program's nature, results may vary significantly among participant. By signing below, the Client recognizes the inherent risk of capital loss and acknowledges that there is no guarantee of achieving their goals through participation in the Program. Any comments made by the Company regarding outcomes are merely opinions. The Company guarantees only that the Services (coaching sessions, recording and notes) and others outlined in this Agreement will be delivered to the Client as specified.
2. I acknowledge that participating in coaching sessions may involve personal challenges and emotional experiences. I voluntarily assume all risks associated with these sessions.
3. In consideration of being allowed to participate in coaching services, I hereby release and hold harmless [Silverlininglash] and [Jasmine Falcon], their employees, agents, and representatives from any and all claims, liabilities, or damages that may arise from my participation in the coaching program.
4. I agree to maintain confidentiality regarding any personal information shared by the coach or other clients during the coaching sessions.
5. If client decides to terminate agreement no refunds will be issued.
6. All material provided and information given to client during mentorship program is copyrighted, for personal use only, it is propriety, any disclosure to a third party is prohibited.
7. I agree and give consent to having the coaching sessions recorded.
8. Any comments, testimonials, photos, recordings, case studies, or success stories you share in the program may be utilized by Silver Lining Lash for marketing and advertising purposes unless requested otherwise. While we may seek permission to use your posts, you acknowledge that anything you contribute to the group can be repurposed for Silver Lining Lash's promotional efforts.
By signing this form and enrolling in this mentorship program, I acknowledge that I have read and understood the terms outlined above and agree to them.
